All of our forms can be completed electronically by following the links below. Once you click the link, enter your full name and email address to begin the process. The system will walk you through all the steps to complete and submit your forms.
If you are having trouble filling out the form electronically, there is an option to print and complete by hand by clicking the button at the top right of the form that says "Fill this out by hand." NEW PATIENT/CLIENT:
Our doctors are not accepting new patients, though there is a short wait list that we can put you on. Dr. Sachs is still accepting new clients for consultation and evaluations (to provide diagnostic clarification and concrete treatment recommendations/referrals).
Please note that the waitlist to qualify for sliding scale is currently at least 6 months.
